Fill in this form and print it out to send your movies to Video Kitchen for conversion to VHS tapes or DVD's. The top part helps you determine what we will charge for all the work you select to be done.

Send no money or credit card information with your order. We will contact you by telephone with our final charges and get your payment instructions at that time.

After processing, your original movies plus any DVD's and tapes will be returned by UPS with $100 insurance to the address below unless you advise us differently. If we determine that the total charges are going to exceed $ we will not run the job without first obtaining your permission. Normal turnaround is two weeks.
